Clinical Analysis of Ileal Perforation due to Typhoid Fever



Abstract
This report was clinical analysis of 44 cases of ileal perforation due to typhoid fever at Department of general surgery, Busan Wallace Memorial Baptist Hospital from Jan., 1978 to Dec., 1982. The results were as follows;
1) There was most predominant in males of 3rd and 4th decades and was prevalent in Aug. and Sept.
2) Roentgenographic studies and blood or stool cultures rather than Widal test were significant in diagnosis.
3) In most cases, there was 1 to 3 weeks duration of illness and operation was done within 24 hours after perforation.
4) In most oases, the site of perforation was within 40 cm from the ileo-cecal valve, the number was single and the size was less than 1 cm in diameter.
5) In surgical management, 2 types of operative procedure were used; First, debridement and primary closure used mostly. Second, segmental resection of the ileum and end-to-end anastomosis was used in-the cases of combined bleeding and more than 48 hours after perforation.
6) In most cases, combined antibiotic therapy with chloramphenicol and ampicillin was good result postoperatively.
